Source only. Lots of masking and blending. A little cloning and some burning. Shadow a simply black oval with a lot of gaussian blur.

If you look closely at the image you can tell where it's been masked, to solve this you could try dodging the outline of the bit that you clone stamped. Other than that, really good! The simplicity of the image just makes it stand out all the more!
