Image done quite a while back, in the PST days.
No other sources used other than the Paris Hilton image. All done with copy/paste, transform, warp, liquefy, cloning and a bit of drawing. I tried to keep the image as realistic as possible.

Only thing I did notice in high res was that her right arm holding her head is got a little evidence showing that you used liquify tool because it's a bit smudged.. I would se the healing patch tool on that or just do some cloning
