Paint ransom over the face with a rough brush.

With a fine brush I added some scars - also used burn tool.

Making the eye look wounded with careful liquified and some burntool. Didn´t want the eye to be the maindamag, this is a guy who could go for one more round if he needed.

With liquify, brush, burntool he´s lips get swollen (also added some teeth).

At this point I think I´ve lost some steps, but basicly worked a lot with burn/dodge tools.
Some of the thin lines I added with burntool. To make the "strangle-marks" I used the method as in step 1 but added noice, burn tool and opacity.

I made a new layer in with I changed his skintone, added some blodish hairline, more roughed blood and burn/dodge som of the scars.

Then I erased some of the over layer to appear differenses in skintone and fade the bloody hairline.
