started with bulb

then i erased the filament using clone stamp tool

added Cigarette with a layer mask
step 4 of 6
added the burning Cigarette head
step 5 of 6
added the skull image
step 6 of 6
before adding smoke i smudge the skull to give smoke effect and blurred it then i added smoke using brushes and eyes to skull and added shadows.....
