Added nails over the fingers.
For the thumb: positioned the nail photo above the thumb then used a layer mask to remove the excess.

Since I only used one nail photo, I added extra nails (from another source photo) and cloned other areas to avoid patterns forming.

Added the background as the lowest layer.
Duplicated it and converted it to a Smart Object. Set the Blending mode to Multiply and added a Smart Filter > Gaussian Blur.
Added Adjustment layers: Black & White and Levels.
Finally, added a gradient set to Linear Burn for the dark/shadow effect.
