i selected the hand and paste it to a new layer"hand". Back to the original layer used stamp tool on the hand to erase it and filled with the floor. While on the original layer ctrl+left click on the "hand" layer and i had the selection of the hand to the background layer. copied to a new layer with ctrl+j and the moved away from the arm. Then used blending mode to the layer.

made a light lamp by using sircle shape and blendig options inner shadow,inner glow with the proper tuning, draw a reflection light and then the main light by a white circle and blening otions again and outer glow.
