The screw consist of a sphere and a dynamic object (spring.)

The head of the screw is the sphere. It was modified by deleting some polys and capping the hole left. To make the Philips Head slots a box geometry with the four polygons around the sides were extruded. This box became the boolean operand "A," which interacted with the transformed sphere to make those slots.

The threads are a spring. The number of turns, size of the wire, and diameter set so that the edges touched. Vertices welded, center polygons deleted and a "cap hole" modifier applied.
Material is Arch and design mental ray.

The pedestal is a "cube" or box with alterations to individual polygons.

For the plate on pedestal the texture were made in Photoshop and placed on a box.
