If you have Blender 3D 2.65, creating a Gear is just a matter of one click. You may have to activate "Extra Objects" to make them. This can be found in the Preferences (Add Curve: Extra Objects) or download the free script at the link I provide below.

So, I created a gear which I've modified a bit, to get something more personal than the standard ones. I just added a cylinder and some planes I put in the middle.

To create my clockwork, I also created some metal pieces, which holds the different gears

And last, I wanted some screws, so I just create a cylinder with cube as boolean. As I just need the screw head, this is enough for my purpose.

For the material, I just created a bump map in photoshop (add noise filter, then motion blur) to get a brushed aluminum effect.

Finally, I put all in scene, and rendered with "Cycles Render".
