I can only explain what I done as I did not make a sbs for it, as it was done for a friend and I did not know I would be entering it in a contest.

First I duplicated the image, and through color adjustments darkend the top layer. Next I erased gently at low pressure the center area of the top image to let the lighter bottom image give a lighting effect. Then went around the outer edges of the lady to darken it. then made color adjustments in skin tone and blended out her second chin area. sharpend the whole image and added noise in touched up spots.
