Cut out horns and skull with path tool.

Desaturated skull,blended hornes with skull.

Made table from source skull, background added.

Made candle and shelf also from source skull. Add picture. Than I was playing with layers styles, burn and dodge tools to add lights and shadows. Made smoke with brush.Used lightning effects for backgground. At final, used levels to adjust whole image.
