Masked out some blocks in the wall with the pen tool.

Duplicated the layer and reduced it a few percent and re-centered it behind the first layer.
Using the burn tool I created the shadows to give it dimension.

Put the screamer behind those two layers.
Desaturated the screamer, then used the liquify to torture the face. I copied some teeth to the bottom of the mouth.

I added my own color back to the screamer keeping the rough sketch feel of the album cover.
