Using elipse tool made an oval and filled with the same color of the skin. Applied some burn inside. That's the head lid.

Added the brain. Made some adjusts on image (copy/paste) to give it a better perspective; changed color.

Copied and pasted the key from supplied source, and added to head lid. Selected the part of the breast and copied, distorted, to make the window. Using pen tool and rounded rectangle tool, drew the lock. Applied bevel and emboss and burn tool.

Duplicated the key; selected half of the heart key ring and the wire, to make a new key ring.

Well, it's late, but she's a bit shy... added flowers and grapevine leaf to hide her... delicacy.

To give her a more human appearance, copied, pasted and masked a girl's face.

Copied and pasted hands.

Duplicated hands layer; between these layers, placed stone heart, changing its color. Erased one hand from top hands layer.
