Pick some clouds from the sky, desaturate them and set the layer mode to "hard light" to make a smoke effect in the jail

In another layer in high light mode add some brush strokes

Paste some jail in the scene and blend them with the clouds

Mask and place the arm, take care with the fingers!

Add some light effects and apply some selective b&w layers to keep only the interesting things in color
