I cut out an eliptical selection from the cap. I was careful to select an area without too much grass so I didn't have to deal with it later.

I then duplicated it twice and rotated the pieces to make sort of a rounded triangle. I masked the individual pieces to blend and then merged the layers into one.

I then cut out the leaves and warped the hell out of them into something I liked.

I then put it together and rotated it into position. To get the chocolate effect, I painted over the section I wanted covered the used a gradient overlay for the shadows and added a highlight layer with a 2.5 px gaussian blur

Added a background with a chocolate to black gradient overlay
