step 1 of 4

select the left cog and put it in a new canvas.

step 2 of 4

duplicate it and flip horizontally.

step 3 of 4

flip the heart shaped chocolate box vertically and place it on the lower right. use warp to tool to shape it further.
place the different elements on the image and use a rectangle filled with white to act as a table.

flatten the image then duplicate it. click 'add a layer mask' then add a filter->blur->guassian blur of 4. then click on the layer mask and use a soft brush w/ low opacity and slowly erase the area in which you want to take away the blur.
