Found this image and used healing brush to fix some the blemishes on her complexion.

Desaturated and dragged to my main image.

Found this image and used it for the hands' source.

Edged hand, used brightness.contrast and liquefy/push tool to fix edges.

Used the mask tool on each note then dragged the mask to the background on a separate pallet, copying each facial feature.

I now have an image like this.

Added a black background and used the burn tool to get a nice mask on the hair.

Final step, removed the woman's face using, pattern stamp, dodge, healing brush, smudge and blur. Added some shadows to the clothes pin to create shadow.
