first i took the jaw of the skull

then i moved the jaw down

then i add some bones to the side

then i add a urinal

then i add the flush mechanism

then i added a light brown color with shadows of the skull and bones and mechanism

then i added some tile. I made this as a pattern first by creating a new document and using a grind then added color and used beval and eboss then i added some noise.

then i added some molding. i made this by using a little darker color of the wall then i used the dodge and burn tool.

then i added some grunge brushes

then i added a dark circle around the whole image
