I keep the part of the hand that I wanted to use, so I masked the fingers.

I masked the foot, as I only needed the toes.

I tried to merge them together. Used the transformation tool to adjust their sizes so they can fit better.

I used the stitches image (thank you again marxus) on every toe.

I created a simple background with some default brushes and gaussian blur.

I used dodge&burn on the nails and toe tips, to give them a bruised - dead look. I also painted around the stitches with purple ad lowered the opacity, for the same bruised effect.
