First thing first cut out the foot i used the pen tool

select a top half of the foot, using a rectangular marquee tool and go to Edit-transform and select perspective, this is how we're gonna widen the young Hulk's foot

well lets go further an fatten the toes a bit, remember its hulk foot! just select, using lessor tool, toes individually and fatten them, using free transform!

Well now toes are exploding, now we are going to make a shoe

Usind a pen tool. make a shape of a shoe

Fill the selection with colour and under custom shape tool, there is a shape called Tile 2. pick that up and it'll automatically places itself on a new layer

duplicate that tile2 shape and rotate it clockwise, at 60 degrees, and you'll get some diamonds shape, then merge the two layers and duplicate until it covers the whole shoe sole.

now load a selection from the shoe sole layer and delete the the unwanted diamond shapes outside the shoe sole. then apply layer stiles, -drop shadow, bevel and emboss, inner shadow and stroke, tweak until something like this happen.

play around with bevel and emboss to soften the shoe sole and create a border around, using the same technique

make a new layer and with a splat brush just splat and load a selection, then on a shoe sole layer copy and paste with that selection

now using kind of a similar brush, erase parts from the shoe sole layer, you should have something like this!

now we are going to make fire, you can start by just making a white rectangular slab

well that slab we made is filled with a black and white gradient, and with smudge tool start dragging upwards and sideways, imitating the rhythm of fire.

duplicate the fire layer once you're happy and resize

to turn our fire hot, we need to go to Hue/Saturation, by pressing CTRL and U, and play with colorizing and eventually fire will get hot

now we hive the foot the identity, by applying the same process that we colored fire with, and this time we just concentrate on turning the foot green! you can see some other fires, and the splash is simple done by some of these splatter brushes on individual layers and merged.

now we let our blanket background show through

now we add some depth on the toes and give'em some volume. by using dodge and burn tool!

now lets make some back smoke using the same technique we made fire with. on a white shape lets use smudge tool and start making some smoke

once you are cool with smoke, apply a blending mode, Difference and magic starts happening, since the smoke needs to be between the blanket and the foot, erase any smoke thats in front of the foot!

Bingo smoke is now smoking, and now we gonna make some kind of a crack on the heel to emphasize the the foot is swelling.

now with the liquify filter go around the crack and drag a bit just to show that something is gonna be coming out soon!

now drop some fire down there! and with dodge and burn tool go around the crack and give the sole some highlights and shadows and well Bingo!!!
