Used the warp tool to play around with the blue gumball to get the basic shape you see here. Also used the burn tool to shadow in the ears.

I added the eyes and tongue
Played around with hue and saturation to get a color I liked. Used the liquify tool to get the shape of the tongue.

I took the head and tongue and copied to new layer then with the copied layer selected I used the blending options I took the fill opacity down to zero and added a drop shadow.
