Starting with the bald head stock, remove it from the background, smooth out the skin surface and remove eyebrows. Form open mouth by cutting out the lower lip and using liquify to form a wider opening. Replace the lower lip and shape to fit the opening that you created. Using the liquify tool continue shaping head to make it appear more gaunt, and bloat the eyes carefully. Use burn and dodge to give the shadows and highlights. Cut open the top of head to make it appear to have hinged top.

To form the snake, take the strawberry texture and work with it in liquify to give the shape then apply dodge and burn to give a more rounded body shape. Using the strawberry that looks a bit like an alligators head, apply it to the body and work together with the smudge tool so that it appears as one. Place so that it is coming out of the mouth.

Using the clouded sky from my stock photo and replacing the sky in the desert cactus scene. By duplicating the cactus scene and cloning out the man and large cactus I was able to create a wider scene that matched the sky.

using the strawberry, liquify to make it fit the shape of the large cactus. place over it and use multiply blend mode

Using the same method as for the cactus, cover the cow skull and place various sizes around background. use burn to enhance cracks in skull.

Using cloning on a separate layer, create some growth in front of skulls to make them appear to actually belong. Use burn tool to create shadows for skulls and floating head.

Finish off by applying an FX mystical color filter and burn a bit of depth in the cut up strawberries inside the head.
