Made selection of tooth and copied onto it's own layer..

Used healing brush and clone stamp to erase other tooth and also to create background for front tooth..
step 3 of 3
Used brush tool for string/cord. and used transform tool to warp it. Also copied and move it down apply blur and reduce opacity to create shadow
